A Manhattan home health-care aide swiped $8,500 from two victims, including an elderly blind woman, law-enforcement sources said yesterday.

Jasodra Mohan, 40, grabbed a blank check from Marian Butler's apartment in an Upper East Side assisted-living facility May 29, police sources said. Butler, who's 92 and blind, became aware of the theft after a $2,000 check written in her name was cashed, police sources said.

Mohan allegedly pulled a similar stunt June 24, snatching a check and a debit card from a 55-year-old woman's apartment in Morningside Heights, which netted her another $6,500.

Police tracked Mohan down on Saturday and charged her with forgery and grand larceny.
